Procedure:
Complete the medical form / questionnaire supplied to you by the Embassy or your employer. It is your responsibility to check that you are in possession of the correct form.
Ensure you answer each question relating to your Iraq Visa Medical in as much detail as possible.
Email us the completed form to info@gpmatters.com prior to the appointment so that we can price the medical and provide you with some available appointments.
Please bring with you:
Photographic ID (ie. passport, driving licence)
A hardcopy of the medical form with the medical questionnaire completed.
Your glasses or contact lenses (if worn)
Copies of any significant hospital letters, abnormal results etc
E-mail us a copy of your vaccination records before the appointment
Locations:
The medical is carried out at GP Matters (24 Buckingham Terrace, Glasgow, G12 8ED). If a chest X-ray is required, it will be carried out at the Nuffield Hospital (25 Beaconsfield Rd, Glasgow G12 0PJ) - 5 min drive from the clinic - we will aim to arrange it for you just after your medical.
After your Iraq visa / work medical:
Please allow 4-5- working days for your Iraq visa / work medical to be finalised. This will then be e-mailed to you and the original documents can be collected at the clinic or posted to you.
Fees - On request
Please e-mail us the medical form/requirements for your Iraq visa / work medical, together with your DoB and role/position.
We will respond the same day with a price and our availability.
